Captive Portal hard timeout does not work

Description

Hard timeout does not work on all three of our pfsense machines.
Running pfSense-2.1.4-RELEASE with squid and squidGuard.
Enabled CP on all three of our pfSense machines.
On all these versions I had and have problems that the idle timeout (58min) and the hard timeout (60min) does not work.
I never tried to hard to find the reason and just restartet CP once a week to diconnect the "unwanted" and old user logins.
So on CP i had to check "disable MAC filtering" on the machine.

Anyomne have any idea how to trubleshoot this?

what kind of list?

It is strange. it works for 2 or 3 days or 3 weeks. you can only see the clients in the hard time out time.
suddenly hard time out does not work. in captive portal you see all clients for example last 3 hours od 3 days, all clients can login without captive portal

captive portal settings:
Enable captive portal is active
Idle timeout = 58
Hard timeout = 60
Disable MAC filtering is active
Per-user bandwidth restriction is active
Local User Manager is active
