The port fields on firewall_nat_out_edit.php should be hidden when protocol isn't TCP, UDP, or TCP/UDP.

Ports were hidden for 'any', 'TCP', 'UDP' & 'TCP+UDP'.
Now only for 'TCP', 'UDP' & 'TCP+UDP'.
