For automatic update checking with monitoring systems like Icinga it would be great to have the update status of the system available via SNMP. This could either be just a boolean like 0 for no or 1 for yes, or something more verbose like the current version installed and the latest version available ...
I am currently comparing parts of the string in iso.3.6.1.2.1.1.1.0 with the contents of http://updates.pfsense.org/_updaters/amd64/version but this doesn't give me information about the 2.3.1_5 release i.e. The _5 part is missing.

There isn't a viable way to do this in base SNMP. You can script up something with the NET-SNMP extend feature without too much effort. Post on the forum if you need assistance.
