• Add enable_peruserbw() to determine whether or not per-user bandwidth fields should be grayed out.
• Call this from enable_change() to set these values when the page is first accessed.

• Add enable_https() to determine whether or not https-specific fields should be grayed out.
• Call enable_https() from enable_change() to set these values when the page is first accessed.

• Modify auth_method_change() to determine whether or not RADIUS-specific fields should be grayed out.
• Call auth_method_change() along with enable_change() to set these values when the page is first accessed.

Properly gray out all fields when captive portal is disabled.

Add pfSense header back. 1.2b6 has moved the header information to fbegin.inc

Add local user manager code imported from Pascal Suter's m0n0wall developers group message.

Add automatic create certificate options to Captive Portal.

Enable/disable Accounting port as needed.

Synchornize with m0n0wall 1.2b3. These files have not really been
modified too much so a straight copy works. 9 files remain to be
patched by hand.

Correctly toggle accounting port if the service is enabled or disabled.
