Fix CP secondary RADIUS auth source GUI

Description
CP in 2.1 supports a secondary authentication source for RADIUS, so you can pick between two authentication sources depending on which fields you have in the portal page. These fields were mis-labeled by a broken commit and are currently called Tertiary and Quaternary server. They should be Primary and Secondary except on the secondary authentication source.
1) They need to go back to Primary and Secondary
2) Need something there in the GUI to clean that up to make it clear that the first Primary and Secondary are for the primary authentication source, and the second are for the secondary authentication source.

So right now we have:
- Primary RADIUS server - Secondary RADIUS server - Tertiary RADIUS server - Quaternary RADIUS server
Is this how they need to be labeled in the GUI?
Primary Authentication Source - Primary RADIUS server - Secondary RADIUS server Secondary Authentication Source - Primary RADIUS server - Secondary RADIUS server
And is this only a GUI change on services_captiveportal.php?
