system.php - Only one entry for DNS Servers

Description
There is only one entry box for DNS servers on system.php and there should be four.
Note that it should be four separate DNS servers each with an associated gateway choice.

Removed conditional that was causing only configured servers to be displayed.
Fixed help/formatting/layout.
