Intermediate internal CA's are created without a reference to the signing internal CA

Description

When adding a new CA in system_camanager.php with the method selected as "Create an intermediate certificate authority", the CA certificate that is created in config.xml does not have a 'caref' attribute to refer to the CA that was selected as the "Signing Certificate Authority". After creation, the CA certificate that was created has "external" displayed as the Issuer on the table of existing CA certificates.
