[feedback form] Include configuration examples for IPv6 WANs

Feedback: Examples would be nice. It seems to me that you could include examples of say some major ISPs to show what the configuration would like if you used those services. This is like reading a math book that explains some math problem without showing any examples. How do you go about identifying what your prefix is?
Note: Unfortunately every ISP is different even "Major" ones can vary by region, there isn't any one good example to show except static. The only way to get the settings you need is to contact the ISP and have them tell you. We can try to make that more clear in the future.

I added a warning to the page informing users that only the ISP can tell them what their settings should be for IPv6, not us.
