Add hostname to Slack notifications

Description
If multiple pfSense instances are using the same Slack channel, there's currently no way to tell which instance is sending that notification going by just the information in the notification. Adding the hostname (or a toggle option to add / keep hidden hostnames) would be a helpful feature.
