Description
If you attempt to add a second authentication server entry with the name of one that already exists, the input validation (rightly) rejects it, however the Descriptive name field is locked and cannot be edited, so it is not possible to repair the error and submit the entry again.

This looks good now, but uncovered another issue on the page, opened that as #5088.
