interfaces_ppps_edit.php: Add PPP interface Country-Provider-Plan does not fill data

Description
In 2.2.5 you could:
1) Interfaces-> (assign) -> PPPs -> Add
2) Link type PPP
3) Select Country, then Provider, then Plan
4) The Phone Number and Access Point Name fields would be auto-filled based on the Country-Provider-Plan selected.
a) In 2.3-ALPHA nothing is fulled.
b) The Provider and Plan drop-down lists are not sorted.

While someone is looking, there are various fields marked as "password", when you try to type in them you don't see what you typed:
gateway
mtu
mru
mrru
They should be some other field type.

Page has been re-written to eliminate the GET logic and replace it with jQuery so that the country/provider/plan and other items update dynamically.
