Add support for user-supplied Host-Uniq tag and handle PADM messages in Netgraph PPPoE

Hi,
can this patch already working be added to pfSense to enable it to deal with custom Host-Uniq tags?
A new input box should also be added to web interface to provide the custom host-uniq.
As creator says:
he custom Host-Uniq tag is passed in the NGM_PPPOE_CONNECT control message, so it can be used with FreeBSD ppp(8) and mpd without any other change.

We’ll pull the support for this in as soon as FreeBSD accepts it. (It’s too big to carry.)

I've created a pull request to my own branch for tests, when stable I'll pull the request to pfSense master.
